Rare drop from Warbringer Mal'Korak, a rare elite who sits on his bloodtusk just outside the Spire of War, at the top of the House of the Chosen in the south-west of Maldraxxus. He is simply there when he is up; nothing has to be done to call him.

Notes and sources
Place: Wowhead's tooltip for the creature has 33.6, 80.0 and 33.8, 79.4; the oldest comment gives /way 33.56 80.73. The wiki has no coordinates and says he sits on his mount just outside the Spire of War. The way up to him runs through hostile elites of the House of the Chosen (comments of 2020 and 2021).
He does not stand there all the time: comments put his respawn at 30 to 90 minutes (2023), 30 to 45 (2024) or 45 to 90 (2022), so a visit may mean waiting; the five minutes on the spot are for the kill alone.
If the corpse is not looted the reins come by mail, and several players found them in the mailbox weeks later (comments from 2021 to 2025), so check the mail before counting a kill as a miss.
About 41 kills are expected at the chance given, and the spread is wide: players on Wowhead report it on the first kill and after 30, 46, 95, 157, 220, 246 and 311 kills. One chance a day per character is how players farm it (comments of 2024 to 2026 speak of killing him daily on several characters); no official statement was found.
No covenant is needed: comments from 2020 on have it dropping for Kyrian, Venthyr and Night Fae characters and for one with no covenant at all (2025), and it can be learned by any of them. Comments of 2021 and 2022 report the mount dropping on characters of level 50 to 53 and being learned at 53; a 2020 one could not learn it at 51.
The wiki lists the Blisterback Bloodtusk and the shield Ruthless Warlord's Barrier as his loot and counts him for the achievement Adventurer of Maldraxxus. Added in patch 9.0.2 (wiki). The item binds when picked up (Wowhead's tooltip).
