Rare drop from Nerissa Heartless, a rare skeleton commander at the House of Rituals in the north-east of Maldraxxus. She needs no summoning and drops it for a character of any covenant; once killed she is back in 40 minutes to an hour.

Notes and sources
Coordinates: the best-rated comment gives 66.03, 35.32, Wowhead's tooltip 65.4-66.0, 34.4-35.6, the wiki 66, 34.4. Other creatures stand around her platform. Nothing has to be done to make her appear.
Players' timings of the respawn: 38 and 40 minutes (2020, 2022), 53 to 55 minutes (2022, 2024) and an hour (2021). So there can be a wait when somebody else has just killed her; the time on the spot is the kill alone. She also drops Necromantic Oil, which Necrolords want for a construct, so she was camped for years. A 2025 comment reports three weeks of looking without seeing her; nothing else read bears that out, and the cause was not established.
About 46 daily kills are expected at 2.18%. The loot is once a day for each character: players track it with the hidden quest 58851 (a 2022 comment), and after the day's kill she comes back as an ordinary elite that drops only junk (comments of 2020 and 2024). Every character has its own daily chance.
Any covenant can get the mount (a 2020 comment: seen dropping for players who are not Necrolords); it has dropped in a raid group (2021) and for a level 57 character (2022). She also counts for Adventurer of Maldraxxus.
