The reins come out of a Cracked Blight-Touched Egg, which is what a Blight-Touched Egg turns into after five real days in the bags. The egg is a rare drop from the flayedwings and galescreamers of Maldraxxus, thirteen kinds of creature in all; the usual place to farm it is the field of Neonate Bonetooths west of the Theater of Pain. No covenant is needed.
Chance 0.19% per try, so about 526 tries on average — Per kill, for the Blight-Touched Egg, which always becomes the mount five days later. Wowhead's page for the egg (read 2026-10-10): 272 eggs in 143,634 recorded loots of the Neonate Bonetooth (0.19%). All thirteen creatures pooled: 832 in 433,593, also 0.19%, running from 0.1% (Galescreamer Elder, 2 in 2,095) to 0.26% (Satiated Sawtooth, 29 in 10,969). The mount's own tooltip shows no chance. wow.gg says 'approximately 0.2-0.3%'

Notes and sources
About 520 kills are expected at 0.19% (1 / 0.0019), with no lockout: every kill is a roll. The time on the spot is one hour's session, not one kill. At roughly 190 kills an hour that is about a 30% chance of an egg per session (1 - 0.9981^190), so two to three hours on average.
The 190 is an estimate from two 2020 Wowhead comments (about 900 kills in roughly five hours; 300 in an hour and a half); nothing newer was found. The Bonetooths respawn quickly, and a 2025 comment found the field roomy enough for two players at once.
Where: Wowhead's tooltip has Neonate Bonetooths between 42.4-45.8 and 45.2-55.6; a top-rated 2020 comment says 'around 43, 50', wow.gg 44, 49. A second place, the Diseased Galescreamers flying at 58.2, 70.9, lost its instant respawn to a hotfix in December 2020 (comments).
No covenant is needed: a commenter looted it as a Venthyr (2025), and the 2020 comments say neither level 60 nor the Necrolords are needed. It still drops in Midnight (2026 comments, the latest on patch 12.1.0).
The egg binds when picked up and has to stay in the bags (wow.gg: not the bank). The reins from the cracked egg are certain by the wiki; Wowhead commenters simply report getting them. A 2026 comment reports an egg whose timer went back to five days after eight or nine; nobody confirms it.
Luck varies wildly. Wowhead commenters of 2025 and 2026 report the egg after 26, under 100, 356, 1,009, 1,391, over 1,500, 2,173, 3,147 and 4,638 kills; one claims 76,136 loots, and a 2026 one suspects there are times when it cannot drop at all. None of that is confirmed.
The thirteen creatures, by Wowhead's count for the egg:
- Neonate Bonetooth
- Flayedwing Fleshripper
- Flayedwing Screamer
- Diseased Galescreamer
- Galescreamer Pup
- Gristlebeak (a rare)
- Ravenous Galescreamer
- Satiated Sawtooth
- Ferocious Nestkeeper
- Flayedwing Matriarch
- Chosen Flayedwing
- Captive Skyripper
- Galescreamer Elder
wow.gg's guide (updated 2026) also names Smorgas the Feaster among the sources, which Wowhead's list for the egg does not.
