Craft it at the mount Synthesis Forge in Zereth Mortis from Schematic: Adorned Vombata, 450 Genesis Motes, a Vombata Lattice and a Lens of Focused Intention. The schematic is in the Grateful Boon, a treasure on a mountain top in the south-west of the zone that only shows once the twelve agitated animals around it have been petted calm; the lens is sold by Vilo in Haven to a character that is Revered with The Enlightened.

Notes and sources
The journal says only 'Protoform Synthesis'. The recipe is 450 Genesis Motes, 1 Lens of Focused Intention and 1 Vombata Lattice (wiki, Wowhead's guide). Nothing here is left to luck except the lattice: the lens is bought, so the slow part is the reputation, about four weeks from nothing by the Anointed Protostag's notes, less for an account that already has it (those notes have the reputation shared by the whole warband since patch 11.1.5). The lens binds to the Warband and is Unique (2) (wiki); the Russet Bufonid uses it too.

The best-rated comment (2022) says the spot is meant for characters that can fly in the zone and gives the climb used above, starting on top of the triangular arch of the nearest vault entrance. Other ways up, from 2022 comments:

- the Dimensional Translator of a Cryptic Grip, which reached far enough only at Firim Research level 5, with one more jump at 40, 78 (a reply)
The animals: twelve in all, in groups of five, four and three (comments name Agitated Cervids and Lupines among them). There is no counter. A pet only counts once the animal lies or sits down, so several tries may be needed (2022, 2023). The one most often missed is the cervid on the rocks in the north-west, at 36.7, 76.1 to 76.4 (2022; a 2026 reply says the tip still holds), and a 2022 comment warns of a lupine on a stone pillar south of Tah Fen.
If the chest was emptied before the mount forge was opened, the schematic lies by itself at the same spot, in front of Tah Fen (Wowhead's guide; comments give 37.0, 78.3). Wowhead's count for the chest has the schematic in 109 recorded loots, with no total given, so no chance can be read from it; guide and comments treat it as certain for a character with the forge open.
Wowhead's guide says the rare reagents of mounts only show up once the Sopranian research and the forge quests are done; whether Vilo sells the lens before that was not established. The Vombata Lattice is used by pets as well, and by the guide such lattices drop from the Dealic research on.
The mount shares its research gate of about 10 days and the forge unlock with the other Protoform Synthesis mounts, and the lattice with the Darkened Vombata and the Curious Crystalsniffer. Guides give two spots for the forge: Wowhead's guide 68.5, 30.1 for the Protoform Repository's cave, comments 70.2, 28.6 for the forge itself, which is used here as for this guide's other forge mounts. A ground mount; the item binds when picked up and asks for level 10 (wiki).
