Craft it at the mount Synthesis Forge in Zereth Mortis from Schematic: Ineffable Skitterer, 500 Genesis Motes, a Tarachnid Lattice and a Protoform Sentience Crown. The schematic is given by the Shade of Irik-tu in Exile's Hollow, who can only be seen and spoken to while the character is dead.

Notes and sources
The journal says only 'Protoform Synthesis'. The mount shares its 10-day research gate and the forge unlock with the other Protoform Synthesis mounts. The Protoform Sentience Crown is the slow part, and no source gives its drop chance.
One guide says the last quest of the 'Finding Tahli' chain only appears after a daily reset. Guides give two spots for the forge, 70.2, 28.6 and 68.5, 30.1; the first is used here, as for the Goldplate Bufonid.
