Sold by Gill the Drill, the A.Z.E.R.O.T.H. vendor among the Time Rift vendors at the Tyrhold Reservoir in Thaldraszus, for 3,000 Paracausal Flakes. The flakes come from Time Rifts, the event that starts there at the top of every hour. The same item can also drop from the boss at the end of a rift and turn up in the rifts' reward boxes.

line and 30 seconds for a portal. A star marks a portal that needs a quest or other unlock done first.
Notes and sources
The journal's 'Cost: 3,000' is Paracausal Flakes: Wowhead's sold-by list has Gill the Drill (NPC 208342, A.Z.E.R.O.T.H Vendor) at 3,000 of currency 2594, and the wiki agrees ('can be purchased for 3,000 Paracausal Flakes from Gill the Drill at the Tyrhold Reservoir in Thaldraszus'). She is the mechagnome from Mechagon, standing in for the A.Z.E.R.O.T.H. timeline. The coordinates are Wowhead's tooltip for Gill the Drill, the same spot as the other rift vendors; no source says outright that the vendors stand there between rifts. The item binds to the Warband (Wowhead tooltip).
The journal names the drop first; the purchase is the route given because the drop's chance was never published. Wowhead recorded 3 of this mount in 1,636 Greater Paracausal Chests (0.18%). Its comments have it from rift bosses of several timelines, not only her own:

The Scourgebound Vanquisher's notes take from Wowhead's guide that every vendor item can drop from the boss of its own timeline; the comments here suggest any rift boss can give it.

- rifts are joined at Soridormi (51.1, 57.1)
