Death knights only. Salanar the Horseman at Death's Breach, in the death knight starting zone (Plaguelands: The Scarlet Enclave), gives it as the reward of Into the Realm of Shadows, the quest after Grand Theft Palomino. Death knights of the allied races and pandaren, who start outside that zone, have the mount when they first log in.

Notes and sources
The journal's 'Trainer: Death Knight' is out of date. No place is given because there is nowhere to travel: only a newly made death knight is in the starting zone, and a death knight that has left it already has the mount.
Making a death knight no longer needs another character of a certain level (wiki: lifted in patch 6.1). Blizzard's 2019 preview says new death knights get the mount after the introductory quests; the wiki says allied race and pandaren ones get it on logging in.
