In the Vision of Orgrimmar (Horrific Visions Revisited, entered from Soridormi's camp south of the Coreway in Dornogal) with at least two Faceless Masks active: clear the Valley of Wisdom, whose boss is Vez'okk the Lightless, then ride the elevator that opens there up to the Orgrimmar Skyway. Killing the Void-Scarred Cubs at the top brings waves of void-scarred wyverns and at last the Void-Scarred Wyvern Matriarch, which drops the reins.

/way is for the TomTom addon. Travel times are estimates: flying at 55 yards a second in a straight
line and 30 seconds for a portal. A star marks a portal that needs a quest or other unlock done first.
Notes and sources
Added with Horrific Visions Revisited in patch 11.1.5. The wiki calls the reins a guaranteed drop from the matriarch; Wowhead has 278 reins in 279 recorded loots of her (99.64%, the figure its tooltip shows under the name 'Void-Scarred Windrider'), so it is counted as certain.
In a group only one player gets the reins from a kill (2025 Wowhead comment, Warcraft Secrets), so each player needs a run of their own.
Masks: Wowhead's guide, the wiki and 2025 comments all say two are enough; another 2025 comment and one seller's page say three.
The elevator is the one inside the Valley of Wisdom, up the ramp on the right when facing the place where the Coagulated Horror stood, and it opens once the valley's objective is done; the other elevators stay blocked. A 2026 comment warns that the elevator can drop a character through its floor at the top, ending the run.

- the cubs: 49.6, 57.0 (Warcraft Secrets; a comment gives 48.7, 54.9 for the wind rider area)
Nothing beyond the Valley of Wisdom has to be cleared, and runs are not limited (wiki), so a failed run can be repeated at once in the same week. The time on the spot is an estimate for one such run; no source gives one.
That the visions are still run in Midnight rests on that 2026 comment and on the notes for the Black Serpent of N'Zoth and the Void-Crystal Panther, which found the wiki describing the Dornogal camp in the present tense and Blizzard saying the visions stay playable. Not established: how they scale for a level 90 character, and whether the two cities still alternate week by week in 2026.
Wowhead's guide (updated 2025) dates from The War Within. Method's guide calls the last enemy the Void-Scarred Windrider; the wiki, Wowhead and Warcraft Secrets call her the Void-Scarred Wyvern Matriarch.
