Drops from Sartharion in the 25-player Obsidian Sanctum when he is pulled and killed with all three twilight drakes, Tenebron, Shadron and Vesperon, still alive. Killed that way he always drops the reins.

/way is for the TomTom addon. Travel times are estimates: flying at 55 yards a second in a straight
line and 30 seconds for a portal. A star marks a portal that needs a quest or other unlock done first.
Notes and sources
The wiki: 'This item has a 100% chance to drop from Sartharion on three-drake hard mode in 25-player Obsidian Sanctum.' Wowhead's tooltip shows 43.35%, which is the share of all its recorded Sartharion loots, kills with fewer drakes alive included. So it is not the chance for a kill done the right way, and the drop is counted as certain; Wowhead's page was not read. This follows the Black Drake, the same kill in the 10-player raid.
The wiki does not say how many reins drop per kill, nor state the lockout. A raid's lockout is weekly as a rule, so a kill with a drake already dead costs a week. Whether the 10-player and 25-player raids lock separately was not checked; if they do, both drakes can be had in one week.
The entrance, beneath Wyrmrest Temple, is from the game's own table. The time on site is an estimate, the same as for the Black Drake. The item binds when picked up and asks for level 27 (wiki).
