Rare drop from any rare creature of Voidstorm. The 14 rares of the achievement The Ultimate Predator make the daily circuit; each can be looted once a day per character.
Chance 2.1% once a day, so about 48 tries on average — Worked out, not read as such: Wowhead's item page records 86 mounts in 57,623 loots over the 14 achievement rares, 0.15% a kill; 2.1% is that figure for one daily circuit of all 14 (1 - 0.99851^14). Over all 21 creatures Wowhead lists it is 128 in 83,835, also 0.15%

About 670 kills expected at that rate, some 48 daily circuits; four players on Wowhead report 197, 216, 282 and 435 kills. The lockout is per character (Wowhead comments), so more characters mean more circuits a day, and comments report drops on characters of level 80 to 89. The same rares drop the Augmented Stormray.

Wowhead also records the mount from seven rare elites outside the achievement (Blackcore, Abysslick, Voidseer Orivane, Nullspiral, The Many-Broken, and Gar'chak Skullcleave and Hardin Steellock in Masters' Perch). They are left out of the circuit because how and when they appear was not looked up.
The 40 minutes for a circuit is an estimate; respawn times were not established. The daily lockout comes from guides and comments, not from an official statement.
