How to get it

Rare drop from Yogg-Saron, the last boss of Ulduar in The Storm Peaks, and only when he is killed with none of the four keepers helping (the fight of the achievement Alone in the Darkness): Freya, Hodir, Mimiron and Thorim must not be asked for their aid before the fight.

Chance 0.98% once a week, so about 102 tries on average — Wowhead tooltip: 0.98%, which is 24 heads in 2,442 recorded loots of Yogg-Saron (item page read 2026-10-10). The same page lists two Titan's Cache objects inside Ulduar with 985 in 98,461 (1.00%) and 17 in 1,720 (0.99%). The counts are per looting player and are not split by how many keepers helped. wow.gg (updated August 2026) says roughly 1%

/way The Storm Peaks 41.5 17.6
From Stormwind (Alliance) about 2.1 min of travel
  1. Take the portal from Wizard's Sanctum to Crystalsong Forest*
  2. Fly the rest of the way
From Orgrimmar (Horde) about 2.1 min of travel
  1. Take the portal from Pathfinder's Den to Crystalsong Forest*
  2. Fly the rest of the way

/way is for the TomTom addon. Travel times are estimates: flying at 55 yards a second in a straight line and 30 seconds for a portal. A star marks a portal that needs a quest or other unlock done first.

Notes and sources

About 100 kills are expected at 0.98%, with one Yogg-Saron kill a week per character (wow.gg).

Bosses that have to die on the way (wow.gg):

  • Flame Leviathan
  • XT-002 Deconstructor
  • Kologarn
  • the four keepers: Hodir, Thorim, Freya and Mimiron
  • General Vezax

Ignis, Razorscale, the Assembly of Iron and Algalon can be left (a 2025 Wowhead comment). The 20 minutes on the spot are a Wowhead commenter's time for the whole run (2025); wow.gg says 10 to 15 minutes. The entrance is from the game's own instance-entrance table; wow.gg's waypoint is 41.6, 17.8.

Sources disagree on raid size: the wiki says the 10- and 25-player versions were merged in patch 7.3.5 and that before then the head dropped only in 25-player mode; wow.gg still says the raid size must be set to 25. A commenter who asked in 2025 whether the size still matters got no answer. Until patch 4.0.3a the head was a certain drop from this fight (wiki).

A second source is the Cache of Timewarped Treasures, reward of the quest Disturbance Detected: Ulduar in Wrath of the Lich King Timewalking weeks, with 103 heads in 16,520 caches on Wowhead (0.62%); comments of 2024 say the keepers do not matter for that one. Players also report the head on characters that entered a saved raid with only Yogg-Saron left (Wowhead comments, 2024 and 2025).

The item asks for level 27 (wiki). The lowest level that may enter the raid was not established, though a commenter got the head on a level 38 character in 2025.

Confidence in this entry: high. Looked up in 2026.
